Plans either provide paper enrollment forms or online enrollment. Please contact your employer for further assistance.

It depends on what your employer decided when they started the plan. Refer to your Summary Plan Description for information.

The federal government limits the amount you can contribute to your account each year. These amounts can also change from year to year. Your plan may also have contribution limitations in place. Please refer to your Summary Plan Description for more information.

You must login to your employee account or complete a paper enrollment/change form with your updated elections and return to ProTPA.

Call our customer service department to assist you in a password reset. Or you can go to Forgot Password/Username to reset.

Once your employer provides your termination date, you may be able to request a rollover/withdrawal online, (if your plan allows for online requests). If your plan doesn’t allow online distributions you may log into your account to access the form; or you can contact your employer or ProTPA for the proper forms.

You can do this on our website or request a new beneficiary form from your employer.

You will need to contact your employer or ProTPA to ensure your plan allows for loans. You may be able to request a loan online by logging into your account; or you may contact your employer or ProTPA to request the proper forms.

A Summary Plan Description gives you the highlights of the rules in your plan. This can be obtained from your employer, or you can call the account manager on your plan to ask questions about the rules.

You can access your account statements on our website. If you prefer your quarterly statement to be mailed, you will need to update your preferences online.
